Registered Nurse Care Coordinator

We are looking for a registered nurse care coordinator who will play a vital role in improving the health and wellbeing of patients living with chronic and complex health conditions across South Western Sydney. 

Key Responsibilities:

• Conduct comprehensive health assessments and support patients in managing chronic conditions
• Deliver care coordination and navigation for at-risk patients enrolled in the program
• Support general practices with care planning, patient enrolment, and program delivery
• Use digital tools to enhance communication between healthcare providers
• Reduce preventable hospital visits through proactive chronic disease management
